I, the owner of the above-named pet, hereby authorize Leander Veterinary Clinic (LVC) to board my pet during the dates listed above. I also hereby authorize LVC to perform the services indicated on the pet information pages while my pet is boarding.

For the safety of our boarders. Every pet who boards with us will receive a Capstar tablet for flea control.

Also, we require dogs to be current on Rabies, DHPP and Bordetella vaccinations and cats to be current on Rabies and FVRCP vaccinations. If I cannot provide documentation showing current vaccination status, I authorize LVC to administer the necessary vaccines.

All boarders must be free of illness at drop off. Owners must be prepared to complete a health questionnaire confirming their pet is free from symptoms such as: coughing, sneezing, nasal discharge, diarrhea, vomiting or lethargy. Leander Veterinary clinic will attempt to notify the owner if the pet becomes ill while boarding. If the owner cannot be reached and, therefore, is unable to inform the clinic regarding measures to be taken, or if the condition of the pet’s health reasonably demands quick action in the opinion of the veterinarian, Leander Veterinary Clinic will administer medical and/or surgical treatment as needed until the owner can be notified.

As the owner of the said pet, I realize that I am responsible for boarding fees and related costs, for the payment of services, and that such services are to be paid IN FULL at the time the pet is discharged.

If the pet is not picked up within five (5) days of the scheduled pick-up date and time, Leander Veterinary Clinic will assume the pet is abandoned. Leander Veterinary Clinic is authorized to remedy the abandonment as prescribed by law.

I further understand that abandonment DOES NOT release me of my financial obligation for services rendered, fees associated with abandonment, collection action and/or legal services.

I understand that should I choose to do leave personal belongings such as blankets, toys, carriers, etc; Leander Veterinary Clinic is not held responsible for lost items.

We will do everything we can to ensure your pet is safe and comfortable while boarding with us.

I understand that Leander Veterinary Clinic does not employ on-site personnel during the hours of 6:00 pm - 6:59 am. All animals boarded at the Facility will be left unattended during those times. By signing this consent form, I agree to release the Facility, its owners, employees, and agents from any and all liabilities, claims or expenses arising from my pet’s boarding stay, including but not limited to injuries, illnesses, or death. I have read and understood the statements outlined in the notice and in this consent form and release. I voluntarily consent to my pet staying at the Facility and acknowledge that I am solely responsible for any consequences that might arise during their stay.
